纤连蛋白、蓝铜胜肽、甘草酸二钾复合物在雄激素性脱发有创性治疗头皮修复中的作用

【摘要】 目的 头皮微针和毛发移植在治疗雄激素性脱发(AGA)中有较广泛的临床引用,这些有创性治疗会导致头皮的医源性创伤,创面修复直接影响疗效。本研究观察纤连蛋白、蓝铜胜肽、甘草酸二钾复合物(FBD)应用于AGA有创性治疗后,在头皮修复中的临床疗效及安全性。方法 本研究采用随机双盲、自身半头对照设计。纳入的患者,根据治疗方式分为毛发移植组和头皮微针组。以矢状面为界将头皮等分为左右两侧,一侧为实验侧局部外用FBD复合物精华液,另一侧为对照侧局部外用安慰剂。随访时对治疗区域头皮进行拍照、皮肤镜检测,通过评估头屑、油腻度、瘙痒、医师疗效评及不良反应等指标,比较FBD在修复头皮创面和微环境中的作用。结果 本研究共纳入患者41例,35例患者完成研究随访,毛发移植组15例,微针组20例。头皮微针组实验终点头皮微环境症状评分实验侧降至接近0分,优于对照侧;且医师疗效评价临床痊愈率为30%,对照侧无痊愈病例(P<0.001)。毛发移植组,头皮微环境症状评分实验侧均优于对照侧,随访终点头屑评分实验侧1.14±1.51,对照侧2.28±1.54 (P=0.001);油腻度实验侧1.6±1.88,对照侧3.2±1.82(P=0.001);瘙痒评分实验侧0.8±1.01,对照侧2.26±1.48(P=0.001);医师疗效评价实验组临床痊愈率达40%,对照侧无痊愈病例(P=0.001)。除头皮微针组1例因治疗中针刺疼痛不耐受退出试验外,未发生严重不良反应。结论 外用FBD复合物能够改善毛发移植术后及微针治疗后的头皮微环境,减轻头屑、油腻和瘙痒等症状,且未见严重不良反应。

【Abstract】 Objective To observe the clinical efficacy and safety of the complex of fibronectin, blue copper peptide and dipotassium glycyrrhizinate(FBD) in the repair of scalp after invasive treatment of androgenetic alopecia.Methods This study adopted a randomized double-blind, self-controlled split-head design.patients diagnosed with androgenetic alopecia in the outpatient department were included and divided into two groups based on the treatment method: the hair transplantation group and the scalp microneedle group.The scalp was equally divided into left and right sides along the sagittal plane.One side was the experimental side, where the FBD complex essence was applied topically, and the other side was the control side, where Placebo(normal saline) was applied topically.Follow-up visits included taking Photos of the treated scalp area and performing dermoscopy tests.By evaluating indicators such as dandruff, oiliness, itching, physician′s assessment of therapeutic effect, and adverse reactions, the role of FBD in repairing scalp wounds and microenvironment was compared.Results A total of 41 Patients were included in this study.35 patients completed the follow-up of the study.There were 15 cases in the hair transplantation group and 20 cases in the scalp microneedle group.In the hair transplantation group, the experimental side of the scalp microenvironment symptom score was better than the control side.At the end of the follow-up, the dandruff score on the experimental side was 1.14 ± 1.51, while on the control side it was 2.28 ± 1.54(P = 0.001); the greasiness score was 1.6 ± 1.88 on the experimental side and 3.2 ± 1.82 on the control side(P = 0.001); the pruritus score was 0.8 ± 1.01 on the experimental side and 2.26 ± 1.48 on the control side(P = 0.001); the clinical cure rate of the experimental group as evaluated by the physicians was 40%, while there were no cases of clinical cure on the control side(P = 0.001).In the scalp microneedle group, the experimental side of the scalp microenvironment symptom score decreased to nearly 0 points at the end of the experiment, which was better than the control side; and the clinical cure rate as evaluated by the Physicians was 30% on the experimental side, while there were no cases of clinical cure on the control side(P<0.001).Except for 1 case in the scalp microneedle group who withdrew from the trial due to intolerable needle Pain during treatment, no serious adverse reactions occurred.Conclusion Topical application of the FBD complex significantly improves the scalp microenvironment after hair transplantation and microneedling, effectively reducing post-procedure dandruff, excessive oiliness and itch.
